Commonwealth Home Support Program

HACC_iconThe CHSP program aims to help the frail aged and people with a disability, through the provision of our services, to remain living independently in their own homes, for as long as it is practical.

The program is jointly funded by the federal and state governments and is auspiced by Gwydir shire Council.

CHSP Staff

Our Staff co-ordinate and implement Programs to assist clients and offer support with a variety of activities and services.  We acknowledge and value our Volunteers, who assist us in the provision of all our client services.   They are very generous people and we could not function without them.

We assist with:

  • Transport Bookings
  • Aged Care Assessment Team referrals
  • Vital Call
  • Meals on Wheels
  • Department of Veteran’s Affairs transport claims
  • Social Functions
  • Social Support

Our clients are important to us.  If we can assist you in any way, please phone us, or call in to the office to speak with us. Confidentiality and a caring, professional approach is assured.
